Abstract:

Background and Aim: Achieving and maintaining a good quality of life is an important goal of diabetes care. Therefore, recognizing the factors affecting the quality of life of these people is important. On the other hand, it seems that people with diabetes will have a chance to live better and improve their quality of life with mindfulness. Also, the implementation of treatment recommendations by the patient, which is referred to as adherence to the treatment regimen, is related to the quality of life of diabetes patients. Therefore, it seems that adherence to the treatment regimen and mindfulness can affect the quality of life of these people. Therefore, the present study aimed to determine the relationship between mindfulness and adherence to the treatment regimen with quality of life in patients with diabetes.
Method: This correlationl predictive study, was conducted for a period of February - June, 2021 with 226 adults with Type 2 diabetes referred to the Baghban clinic of Mazandaran University of Medical Sciences, continuously and based on inclusion criteria: having an age of 18 to 65 years, Reading and writing ability, no pregnancy, no psychiatric disorder, no severe visual or hearing impairment. Data were collected using a demographic characteristics questionnaire, Kentucky Inventory of Mindfulness Skills(KIMS), The Diabetes Activities Questionnaire (TDAQ), Diabetes Quality of Life Short Form(DQOL). Due to the covid19 pandemic, the questionnaires were provided to the research participants in Google form and through WhatsApp and Telegram software. Data analysis was performed in SPSS software using descriptive statistics and Pearson correlation coefficient and regression analysis at a significance level of 0.05.
Results: The results showed that the majority of research participants were female (61.9%) and 74.8% were in the age range of 45-65 years. Total score of quality of life 53.23 ±8.815 with range of scores (26-70), total score of mindfulness 137.23 ±18.03 with range of scores (60-181), total score of treatment adherence 903.71 ±259.64 with the range of scores (20-1300). The results of correlation between variables showed that mindfulness had no significant relationship with quality of life (p = 0.60), but had a positive and significant relationship with adherence to treatment regimen (p <0.001, r = 0.36). to the treatment regimen Adherence had a positive and significant relationship with quality of life (p <0.001, r = 0.47). Also, the results of simple linear regression test showed that the variable of mindfulness can’t predict the variable of quality of life (p = 0.60). But the variable of treatment regimen Adherence can predict the variable of quality of life (p <0.001) (B = 0.01). Also, the results of multiple linear regression test of all variables showed that the only variables remaining in the model were the lifestyle dimension components of treatment regimen Adherence (p <0.001) (B=0.01) and Acting with Awareness dimension components of mindfulness (p = 0.001). (B = 0.30) and among the demographic variables, economic status (p <0.001). (B =3.45). So that they can predict 34% Variences of quality of life in people with diabetes.
Conclusion: Based on the results, 34% of the changes in the quality of life of diabetics are predicted by the concepts of lifestyle, Acting with Awareness and the economic status
variable. The results of this study can expand the current knowledge regarding the factors affecting the quality of life of people with diabetes. It also contributes to future research for improving the quality of life in this population. According to the results of this study, to improve the quality of life of diabetics, care and interventions can be provided to these people, which leads to improved performance Acting with awareness and lifestyle. Accordingly, the present study provides a basis for examining whether mindfulness-based interventions should be included in the diabetes care program in addition to lifestyle education; Which may be a way to improve the quality of life in this population. Also, considering that the economic status is plays more role than two mentioned variables, in predicting the quality of life of patients; It is recommended that policy makers consider this issue in decision making for providing care of these patients and try to help improve the quality of life of these patients by reducing care costs and increasing the availability and cost-effective services. Therefore, applying the findings of this study to holistic diabetes care can facilitate the achievement of an important outcome of diabetes care, which is the improvement of quality of life.

Alice Khachian, (PhD, MED, MSN, BSN), Professor and faculty member of Iran University of Medical Sciences appears to be a researcher with publications in areas such as educational management, educational leadership, and adult education. One of her recent publications is related to 'factors affecting change management'. She has also been involved in data analysis and literature reviews, as indicated by a 2024 publication in which she participated in data analysis and design. Her research interests span across nursing, education, and chronic disease patient care. Iran University of Medical Sciences is one of high ranked Medical Universities in Iran. As one of the most influential schools of nursing and midwifery in Iran, the School of Nursing and Midwifery of Iran University of Medical Sciences with over 40 years of experience in admitting students of nursing and midwifery is honored to train graduates of these disciplines to perform managerial, educational, and therapeutic roles.
