Patient screening is critical. In its most basic form, it is inquiring about patients' symptoms before seeing them in a therapeutic environment. You can accomplish this by having the patient fill out a form or interviewing them in person or over the phone. Patient screening, when done correctly, has the potential to minimise hospital readmission rates. The 30-day readmission rate for patients with chronic obstructive pulmonary disease (COPD) and congestive heart failure (CHF) reduced to 2.34 percent when one hospital introduced patient screening. That's a considerable increase when you consider the national average for COPD patients is 10 to 20% and just under 10% for CHF patients. Patient screening is also essential for delivering general care, whether it is routine or not. To treat patients successfully, healthcare practitioners must understand the symptoms they are experiencing as well as their general medical history. When patients arrive at a healthcare centre, traditional paper and ink forms are employed. Patients may be asked to mark regions on a diagram where they are in pain, check boxes that identify their symptoms, and provide their medical history. While paper forms allow patients to express their problems in their own words, they can also lead to mistakes. For example, the patient's handwriting might be unreadable, or the staff person putting the information into the clinic's electronic medical record (EMR) system could make a mistake. Before the pandemic, several healthcare professionals started adopting electronic check-in methods.
